Summary of changes - addenda

  1. PLACEMENT

For BPVC addenda, the summary of changes page is the first page after the copyright page. No other front matter elements may run in with the addenda summary of changes page (back should be blank).

  1. PAGE NUMBER (FOLIO)

Number the pages of an addenda summary of changes starting with an italic (c) enclosed in parentheses, followed by pages (d), (e), etc.

  1. INTRODUCTORY PARAGRAPHS

Three paragraphs precede the summary of changes entries. See Sample. The statement includes

  1. an explanation of how replacement pages work

  2. a paragraph explaining which margin designator corresponds to each addenda

  3. an explanation of the record number listed in the Change column

If the addenda contains no revisions, the following statement replaces the second paragraph, and the third paragraph is omitted:

No revisions are contained in Section XX of this Addenda.
